Riverdale processes personal data about its patients and is a ‘Data Controller’ in respect of this for the purposes of the Data Protection Act 1998.

This data may be written or held on computer systems and will include:

  • Details about treatment, care and support that you need and receive
  • Results of investigations, tests and previous medical treatment
  • Relevant information from other health and social care professionals, relatives or those who care for you and know you well.

How your information is used

Your information helps us to manage the care and treatment you receive to make sure that:

  • Everyone involved in your care has accurate and up-to-date information to assess your needs and provide necessary and relevant treatment.
  • Full information is available should you need additional medical treatment or be referred to an external medical department.
  • Upon discharge, ongoing care and support is put in place.
  • Your concerns can be fully investigated if you have need to complain.

Why do we hold this information?

  • To ensure that we look after you and your health from assessment to discharge.
  • To prepare statistics on performance and review our services.
  • To train and teach health and social care professionals.
  • To audit our accounts and services.
  • To investigate complaints or untoward incidents.
  • To conduct research and development.

How do we keep your information confidential?

Everyone involved in your care has a legal duty to keep information about you confidential and access to it is strictly controlled. We will uphold your right to confidentiality unless we have a legal duty to release information or we have obtained your permission to do so.

We will need to pass on information to your GP regarding your health, diagnosis and treatment. We may also be required to pass this information to other individuals or services who may become involved in your care. We only pass on information about you if there is a genuine reason to know. Anyone who receives this information is also under a legal duty to maintain confidentiality.

We will not share your information with third parties except under exceptional circumstances, such as when the Health & Safety of others is at risk or where the law requires us to do so.

Access to your records

The Data Protection Act 1998 states that you may access the information we hold about you. You can request this by writing to the contact below. Please note that under the provisions of the Data Protection Act 1998 we may charge a processing fee not exceeding £50.
